Aging Well Program
This program, led by a Registered Nurse, provides support to at-risk seniors through system navigation.
- Description: Provides support to at-risk seniors through system navigation, and early identification, treatment, and care coordination of patients with cognitive impairment.
- Objectives: Improved care coordination and promotion of quality patient care and safety in the community.
- Eligibility: Patients > 65 years with established or possible diagnosis of dementia, or frail elderly in need of support.
