The Alliston Family Health Team (AFHT) is an interprofessional primary care organization dedicated to providing high-quality, comprehensive health services to the residents of Alliston and the surrounding area. We believe in a collaborative approach, working closely with local physicians and Nurse Practitioners to support your health journey. Our team provides preventative care, chronic disease management, health promotion, and mental health support, focusing on empowering our patients to achieve and sustain their health goals.
